FAQ: OCS: How do I remove a grade of 4.0?


  • Go to the Office of the College Secretary (OCS) and inform the student records evaluator (SRE) of the subject in which you got a grade of 4.0.
  • You will then be issued a removal permit by the SRE.
  • Have your removal permit signed by the instructor of the subject to be removed. You will also be informed by your instructor regarding the schedule of the removal exam.
  • If you will take the removal exam outside of the regular removal period, bring the removal permit to the cash division at the Administration building. Pay the required amount. You will be issued an official receipt after paying.
  • Return to the Office of the College Secretary and have your removal permit approved. Present your official receipt, if any.
  • The OCS staff will re-issue the approved removal permit together with the removal grade sheet form.
  • Fill out the removal grade sheet form with the required information.
  • Before the removal exam, present the removal permit and the removal grade sheet form to the instructor of the subject to be removed.
  • Get the result of the removal exam from the Office of the College Secretary. Processing period is 5 working days after the removal exam.
